Advance care planning (ACP) is not just about old age. At any age, a medical crisis could leave someone too ill to make his or her own healthcare decisions.

ACP involves learning about the types of decisions that might need to be made, considering those decisions ahead of time, and then letting others know about care preferences, often by putting them into an advance directive. Conversations about these decisions start with those providing care.

Join nurses Jeri Lynn Ashley, MSN, RN, AOCNS, CHPN, and Sally K. Aldrich, RN MSNA, for three On-Demand Learning opportunities focusing on ACP. During the three 30-minute recorded webinars, they share details on how primary care providers can initiate conversations about advance care planning and capture the information needed for new billing codes.

Each session features case studies, examples and links to relevant tools and resources. To access the sessions, start by selecting the topic that most interests you. All sessions are available 24/7 – at work, at home or on-the-go!
